Attention all Addison Panther Student Athletes in grades Kindergarten through eleventh grade, Addison Community Schools will be hosting a sports physical day on Wednesday, May 25 in our Auxiliary gym from 10:00 AM - 5:00 PM. The cost for the physical is $10. This physical will count for any and all sports played by your student athlete during the 2022-2023 school year.
Please send your student to school with the physical form filled out correctly and signed by the parent or guardian.
Parents do not need to be present for the physical to take place.
